All Facilitators MUST call Monique Chavez at (505) 277-5800 BEFORE beginning any Mind in the Making Learning Program.

Monique will conduct a phone survey and begin the process of registering your session of Mind in the Making, ordering participant manuals, sending out a Facilitators Packet, and forwarding your information to Ellen Biderman, the Mind in the Making point person in our department.

Again, email (or call) Monique in order to obtain the most current Attendance Roster and the Participant Guide order form! mchavez@unm.edu

Upon Completion of your Mind in the Making Modules, Monique will need you to turn the following: your attendance roster (typed and in alphabetical order); all course evaluations: 2 sets of the self-assessment (which should be completed midway and at the end of the program) and the final course evaluations; and, all participant registration forms. Once all these forms are turned into Monique, she will send you the official Mind in the Making certificates.

Be sure to contact Monique BEFORE beginning Mind in the Making and she will register you as facilitators and walk you through any requirements and forms necessary for students taking the course for credit.

Also, please do not create certificates for participants. Monique will issue certificates at the end of the course either for course completion or training hour earned.

Ellen can send you UNM Extended University registration forms as need be. Be sure to assign participants taking Mind in the Making for credit a letter grade and send Ellen a list of these grades via email so that she can send them in to UNM  records.
